Patient Registration Details

On your first visit to the London Clinic of Dermatology you will need to fill out four forms:
  • Patient Registration Details
    Your personal details for registration at The London Clinic of Dermatology, including your contact details and those of your FP or referring doctor.
  • Confidential Medical Questionnaire
    To enable us to provide the best and safest possible medical treatment, we require you to give us full details of your medical and family history.
  • Patient Responsibilities
    Designed to demonstrate that mutual respect and co-operation are basic to the delivery of quality health care service. Your responsibilities when you are a patient of The London Clinic of Dermatology.
  • Financial Policy
    We are committed to providing you with the best Dermatological care available. We like you to read the statement of our Financial Policy prior to treatment so there are clear expectations.
You are welcome to complete and bring these forms with you when you visit the Clinic. All forms are downloadable below in PDF format which you can complete online, print, sign and bring to your appointment.
